Job Summary:
Evaluates and provides family oriented services to the home health patient with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments; fine motor, sensory motor, and developmental delays; as well as traumatic injuries and swallowing disorders. This includes patient and caregiver education; discharge planning; and consultation to other professionals, clinics or agencies.
Essential Functions:
PATIENT CARE
1. Responsible in performing initial and ongoing comprehensive assessment of the client's needs, including Outcome and Assessment information Set (OASIS) assessment at appropriate time points.
2. Responsible in initiating the plan of care and revising as necessary. Initiates the plan of care and makes necessary revisions as patient status and needs change.
3. Completes an initial assessment and history of current and previous illness(es).
4. Regulatory re-evaluates patient nursing needs.
5. Provides home health services and/or treatment requiring substantial and specialized nursing nursing skill. Uses health assessment data to determine nursing needs.
6. Counsels the client and family in meeting nursing and related needs.
7. Initiating appropriate preventive and rehabilitative nursing procedures.
8. Preparing clinical and progress notes. Evaluating outcomes of care. Planning for the discharge from services. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
9. Coordination of services and referral of other services as needed.
10. Maintain communication with physicians, family members and other agency personnel to adequately plan for and provide the patient's needs. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
11. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
12. Provides health care instructions to the patient as appropriate per assessment and plan of care.
13. Acts as Case Manger when assigns by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
14. Supervising LVNs, CHHAs and paraprofessionals providing services.
15. Attend and participates in Continuing Education programs.
16. Adheres to all Company Policies and Procedures.
17. Demonstrates flexible and efficient time management techniques.
18. Prioritize workload so as to meet productivity standards and applicable deadlines.
19. Maintains strict confidentiality with regard to protected and sensitive information.
COMMUNICATION
1. Prepares clinical notes and updates the primary physician when necessary and at least every sixty (60) days.
2. Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition, obtains/receives physician's orders as required.
3. Communicates with community health related person to coordinate the care plan.
ADDITIONAL DUTIES
1. Participates in on-call duties as defined by the on-call policy.
2. Ensures that arrangements for equipment and other necessary items and services are available.
3. Instructs, supervises and evaluates home health aide care provided every two (2) weeks.
Knowledge, Skill and Experience:
1. Minimum Education required, Graduate of an accredited school of nursing.
2. Current licensure in State California Board of Nursing, CPR certification and valid driver's license.
3. Minimum Experience required: Minimum of one (1) year recent clinical experience in an acute care setting or an institutional setting preferred. One(1) year experience in Home Health preferred.
4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist.
5. Must be able to maintain strict confidentiality with regard to protected and sensitive information. Must be able to solve problems independently and confidently by applying analytical and logical thinking.
Physical Job Description:
Typical Working Conditions: Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
Equipment Used: Computer keyboard, telephone, fax machine, copier.
Essential Physical Tasks: Ability to lift approximately 15-20 lbs.
