Optimae LifeServices is accepting applications for a full-time Intake Coordinator/Trainer to serve Johnson, Washington and Iowa Counties. The Intake Coordinator/Trainer will be responsible for overseeing all incoming referrals for site services and other programs as needed (community integration, community services, etc.). Coordinates the onboarding of new customers into the site program or other programs as needed. We are seeking an individual who works well in a team environment, is self-driven and highly organized.
Duties include:
- Aptitude for basic computer skills.
- Communicate with customers, co-workers, staff supervised, the public, and other providers of service.
- Maintain close communication with referral sources including IHH workers. Case Managers, local hospitals, and other outside sources of referrals.
- Honor and respect the rights and privacy of our customers.
- Attend Residential Options on a biweekly basis each month.
- Maintain an organized system for record keeping on all incoming referrals.
- Attend agency referral meetings at least once a week.
- Coordinate with the designated Service Coordinator a move in date for new customers.
- Must display the aptitude and willingness to work closely with the staff and customers, excellent communication and networking skills, and use initiative, good judgement, and leadership skills.
- Work as a leader of a team.
- Work in a stressful environment.
Qualifications:
- High school diploma or GED is required.
- Bachelor's degree from an accredited college or university is preferred.
- Experience preferred in the field of human services.
