Optimae LifeServices in Lee County is currently accepting applications for a full-time Service Coordinator with office located in either Keokuk or Fort Madison. This is a salaried position that requires On-Call and possible weekends. The ideal candidate will be highly motivated with proven experience in the planning and implementation of ID/BI waiver and Habilitation services.
This position requires an individual that is able to work in a fast-paced, frequently changing environment. We are looking for a leader that demonstrates good communication skills, interpersonal skills, the ability to coach direct reports, model leadership and build teams. Service Coordinators must possess the ability to manage a flexible schedule. If you are looking for a job that provides variety every day – this is the role for you!
Duties:
- Complete new customer intakes including completion of assessments.
- Create and update customer records.
- Participate in customer team meetings.
- Create Individualized Service Plans for each customer on caseload
- Create and implement customer specific behavioral plans.
- Provide direct skill teaching to Optimae customers, as needed.
- Monitor service delivery
- Supervise Direct Support Professionals (DSP)
- Develop staff through training, monthly coachings and team meetings
- Manage direct reports payroll.
- Participate in on-call rotation (some weekends and holidays may be required)

Qualifications
- Be at least 18 years of age
- Licensed driver with automobile insurance and a working vehicle
- Background check required which includes Child and Dependent Adult Abuse and driving record check
- Three (3) years experience in the Mental Health field
- Must have supervisory experience and demonstrate great leadership skills
- Degree preferred but not required
