Optimized Senior Living is a non-traditional assisted living facility based in Lebanon, OH, with a new location in Kettering, OH. With a new location, comes new opportunities! We are in need of a full time Director of Nursing.
DIRECTOR OF NURSING
The Director of Nursing is responsible for the below responsibilities. The DON is also agreeing to be available by phone or text to staff at all times. When not available, they agree to respond within 30 minutes, or notify upper management they will be absent so there is appropriate coverage.The DON agrees to take on call when needed based on the need of the facility that could include all shifts or staying late to ensure staffing on the floor.
Responsibilities:
- Assist residents with activities of daily living (cleaning, grooming, eating, transporting, bathing, etc.)
- Implements and is an example of OSL Core Values in all aspects of work
- Be available for phone calls when not at OSL and effectively communicate within 30 minutes of receiving phone calls.
- Maintains nursing operations by initiating, coordinating, and enforcing program, operational, and personnel policies and procedures.
- Orient and train caregivers on medications and disaster plans
- Coordinating with the other nursing managers to assign appropriate tasks and responsibilities to staff to exceed resident care needs..
- Maintains staff job results by coaching, counseling, and disciplining employees; planning, monitoring, and appraising job results.
- Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
- Promotes resident’s independence by establishing, implementing and reviewing Resident care plans and assessments.
- Provides information to residents, family, and health care teams by answering questions and requests.
- Resolves resident needs by utilizing multidisciplinary team strategies with the utmost respect and compassion.
- Maintains safe and clean home by implementing standard procedures, and rules
- Protects patients and employees by developing and interpreting infection-control policies and protocols; enforcing accurate medication assistance, storage procedures, and controlled substance regulations.
- Maintains documentation of resident care services and staffing regulations by auditing resident records.
- Maintains a cooperative relationship among all care teams by communicating information; responding to requests, accommodating resident needs; building rapport; participating in team problem-solving methods.
- Review medications as they arrive & appropriately update in ALIS
- Coordinate room turn over for new residents
- Perform daily rounds on residents and accommodate resident needs
- Complete the Post Mortem Protocol
- Coordinate all care with skilled and hospice for residents
- Medication management for all residents
- Coordinate with pharmacy
Auditing ALIS for missed meds, and the med carts. Document appropriately and take action.
- Assist with policies and implementation of nursing protocols, nursing meetings, education.
- Assist with training new employees on medications, and how to properly assist with them and document in ALIS for medications - assist with orientation
- Regular trainings on needed inservices
- Educating/supervising staff and disciplining as needed
12 Month Success Tracking
- To have happy and satisfied residents and families with communication and care, based on surveys and comment cards we receive throughout the year
- To have tenure with employees – minimize turnover. When a problem arises with an employee, it is handled appropriately, and the situation is evaluated immediately to minimize risk of same issue occurring in the future.
- Communication rating a 10 with the Executive Director. The rating will be based on peer evaluations.
- Keeping accurate and up to date records of residents and employees. This will be measured come survey time when all these documents are needed.

Requirements:
LPN or RN in State of Ohio
Clear background check
Reliable Transportation
At least 2 years of management experience in any field
