Job Description:
We are seeking a dedicated and compassionate Career & Family Services (CFS) Case Manager to join our team. As a CFS Case Manager, you will take on a dual role of being responsible for the client's service coordination as well as providing direct support to the client in activities of daily living (ADLs).
Key Responsibilities:
- Provide case management services, including coordinating and monitoring services and supports.
- Provide direct support to the individual in all areas of daily life, including food preparation, bathing, restroom use, medication administration, and socializing.
- Conduct assessments and ongoing evaluations of the client's needs.
- Collaborate with other service providers, community organizations, and agencies.
- Develop and implement individualized service plans (ISPs) in collaboration with the client and their circle of support.
- Advocate for client's needs to ensure they have access to necessary resources.
- Maintain accurate, up-to-date case records and other documentation.
- Participate in training and professional development activities to stay current with best practices and regulatory requirements.
Qualifications:
- Associate's degree or higher.
- Must be able to pass both a criminal DOJ/FBI LiveScan background check and DMV check.
- Valid California driver's license and insured vehicle.
- 1 year experience working with people with developmental and intellectual disabilities.
- Valid CPR/First Aid certification.
- Previous experience in case management or a related field. (Preferred)
- Working knowledge of state and local regulations related to case management and family services, or willingness to learn.
- Strong written and verbal communication, organization, and problem-solving skills.
