Job Overview
The Wraparound Facilitator plays a crucial role in supporting individuals and families by providing comprehensive, individualized care coordination. Duties
- Collaborate with clients, families, and multidisciplinary teams to create and implement individualized wraparound plans.
- Facilitate meetings with stakeholders to discuss client progress and adjust care plans as necessary.
- Provide guidance and support in areas such as art therapy, gender issues counseling, and psychopharmacology.
- Monitor client progress and outcomes, ensuring that all services are delivered effectively and efficiently.
- Advocate for clients’ needs within the community and connect them with relevant resources.
- Maintain accurate documentation of client interactions, treatment plans, and progress reports.
- Participate in ongoing training and professional development to stay current with best practices in wraparound services.
Requirements
- A degree in social work, psychology, counseling, or a related field is preferred.
- Experience in providing wraparound services or case management is highly desirable.
- Strong understanding of mental health issues and community resources available for clients.
- Skills in art therapy, gender issues counseling, or psychopharmacology are considered a plus.
- Excellent communication skills, both verbal and written, to effectively engage with clients and stakeholders.
- Ability to work collaboratively within a multidisciplinary team environment.
- Strong organizational skills with the ability to manage multiple cases simultaneously.
