Options Residential offers rehabilitative residential services to adults with brain injuries, mental illness, and/or other disabilities. Our primary goal is to work with our individuals to help them meet their goals for independence.
We have an exciting opportunity for you to join Options Residential as a Community Supports Specialist! This is a full time, flexible schedule position, where you will drive to meet clients in their homes to work with them.
What does a Community Supports Specialist do?
The Community Supports Specialist is responsible for maintaining quality of services by providing, supporting, and evaluating service delivery for individuals receiving residential services for mental illness or brain injuries. This person is responsible for providing on-call and in person service crisis intervention, counseling, and support via phone, virtual visits, and in-person visits to our individuals receiving services.
You'll be meeting regularly with clients on your caseload, for about 6 hours each work day. There will be about 10 hours built into your workweek for flex time to manage crisis phone calls and situations.
Main duties include:
- Monitoring programs for individuals served to ensure the programs support the mission and vision of the organization and are compliant with licensing rules and regulations.
- Leading programs assigned with a Person-Centered Rehabilitative philosophy that promotes the ability for individuals supported to be involved in their own recovery and to achieve the best quality of life possible.
- Developing appropriate and professional relationships with case and mental health managers, consultants and other members of the support team.
- Managing the 24-hour telephone crisis line and respond to any emergency calls from our residents.
- Delivering community-based supports according to the level of service being provided.
- Maintaining regular contact with individuals supported, staff members, family members and attend key meetings to support the implementation of program planning.
- Coordinating consulted services such as, nursing, specialist services, physicians, psychologists, ARMHS and all other consultants ensuring resolution of outstanding issues.
Salary: Hourly position, $23-25 hourly DOQ.
Qualifications:
- Bachelor's degree in a related field (e.g., social work, psychology, human services), and at least one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or a combination of education level and equivalent work experience.
- Experience in program management, crisis intervention, or related roles.
- Availability to be on call 24/7 for emergency assistance
- Strong communication and organizational skills.
- Knowledge of relevant 245D regulations and licensing requirements.
Options Residential, Inc will conduct a Minnesota Department of Human Services Background Study on all new employees. Employees in driving positions will also be subject to a Motor Vehicle Background check. Continued employment will be contingent on a cleared background study and a satisfactory MVR.
