What does a Community Supports Specialist do?
The Community Supports Specialist plays a vital role in enhancing the quality of care for individuals receiving waivered services, ensuring their unique needs are met with the utmost professionalism and compassion. This position involves conducting weekly check-ins through phone calls, virtual visits, and in-person meetings to provide ongoing support and foster well-being. The role incorporates an on-call component, where you will assist individuals with problem-solving, provide timely crisis intervention, offer valuable guidance, and deliver essential moral support as needed.
In this role, you'll meet regularly with clients on your caseload for approximately 6 hours each workday. Additionally, you'll have about 10 hours of flex time built into your work week to manage crisis calls and respond to urgent situations.
Main duties include :
- Monitoring programs for individuals served to ensure they align with the organization's mission and vision, while also adhering to licensing rules and regulations.
- Building a rapport through a person-centered approach empowering the individual to take an active role in their recovery and work toward achieving the highest possible quality of life.
- Establishing and maintaining professional, collaborative relationships with case managers, mental health professionals, consultants, and other members of the support team.
- Overseeing the 24-hour telephone crisis line and promptly responding to emergency calls from individuals as needed.
- Providing community-based support tailored to the specific level of service required for the individual.
- Maintaining regular communication with individuals served, staff, family members, and attending key meetings to ensure effective implementation of program plans.
- Coordinating consultative services, including nursing, specialist services, physicians, psychologists, ARMHS, and other consultants, to ensure timely resolution of any outstanding issues.
Starting Salary Range : Hourly position, $23-25 hourly DOQ.
Benefits : Eligible 1st of the month after 60 days employment. Health insurance (ICHRA), Dental, Vision, Company Paid Life / LTD. 401k-eligible after 90 days employment. Must be scheduled 30 hours per week to be benefits-eligible.
Qualifications :
Options Residential, Inc will conduct a Minnesota Department of Human Services Background Study on all new employees. Employees in driving positions will also be subject to a Motor Vehicle Background check. Continued employment will be contingent on a cleared background study and a satisfactory MVR.
