What are the responsibilities and job description for the Chaplain/Bereavement Coordinator position at OpusCare?
Participates as an active member of the interdisciplinary team identifying and addressing spiritual matters through:
· Case consultation and case finding.
· Collaboration in interdisciplinary team meeting.
· Patient/family visits.
· Establishment of a spiritual care plan.
Coordinates spiritual services within structure of a team.
Assesses spiritual factors related to patient care and intervenes appropriately.
Documentation should include patient’s religious preference and assistance offered.
Provides spiritual support of patient/family on individual group basis. Support services must be organized to meet the needs of patients, families, and others that may have vital interest in the patient’s wellbeing. The services must conform to the plan of care. Patient/family records should indicate the extent to which support services are used. The plan of care should be consistent with the patient/family needs.
Provides spiritual counseling to meet needs in accordance with the patient’s and family’s acceptance of this service, and in a manner consistent with patient and family beliefs and desires.
Provides Team Manager with weekly schedule of patient appointments and follow-up documentation.
Provides explanation of spiritual aspects of care to interdisciplinary team members.
Acts as liaison with professional religious community by making reasonable efforts to arrange for visits of clergy and members of religious organizations in the community to patients who requested such visits.
Administers sacraments (or arranges for them to be administered) as requested and performs funeral or memorial services upon request by the family.
Educates religious community to the collaboration of Hospice and clergy.
Develops and assists with educational services in the community.
Develops, supervises and evaluates the annual Hospice Memorial Services.
Assumes other responsibilities as requested by the Supervisor.
Other duties related to bereavement and pastoral care.
Coordinates contact with the bereaved within appropriate time frames as indicated on the bereavement assessment and bereavement plan of care.
Provides coordination of bereavement support and services to family and caregivers of hospice patients.
Availability to work across the Service area.
Other duties as assigned by supervisor.

Pay: $28.00 - $31.00 per hour

- Bachelor's (Preferred)
Experience:
- Hospice care: 1 year (Preferred)
- Chaplain: 1 year (Required)
