We are seeking a dedicated and results-driven Hospice Community Outreach Representative to drive patient admissions and expand our reach within the local healthcare community. This role focuses on identifying, establishing, and maintaining relationships with physicians, hospitals, nursing facilities, and other referral sources to generate new patient referrals and ensure that those in need receive compassionate hospice care. The ideal candidate is a motivated self-starter with excellent communication and sales skills who is passionate about educating the community on the benefits of hospice care.
Key Responsibilities:Business Development & Referral Generation:- Actively market hospice services to local physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare providers.
- Develop and implement strategic outreach plans to drive patient referrals and increase census growth.
- Regularly meet with referral sources to educate them on the hospice program, services, and benefits.
- Establish strong professional relationships with healthcare providers, social workers, case managers, and community partners to increase awareness and referral volume.
- Work closely with referral sources to identify eligible patients and ensure a smooth transition into the hospice program.
- Provide timely follow-up on leads and referrals to facilitate prompt admissions.
- Serve as a trusted resource to families, caregivers, and healthcare professionals, guiding them through the hospice admission process.
- Sign consents and explain services to families.
- Represent the hospice at community events, health fairs, and networking functions to enhance visibility and educate the public.
- Organize and participate in educational sessions for physicians, hospital staff, and community partners about the benefits of hospice care.
- Create and distribute marketing materials to promote hospice services effectively.
- Work closely with the clinical and admissions teams to ensure seamless communication and transition of new patients.
- Track and report referral trends, outreach efforts, and admissions progress.
- Provide feedback and insights on market conditions and referral challenges to improve strategy.
- Proven experience in healthcare sales, marketing, or community outreach, preferably in hospice, home health, or senior care.
- Strong networking and relationship-building skills with local healthcare professionals and facilities.
- Excellent communication, presentation, and negotiation abilities.
- Self-motivated with the ability to work independently and meet referral/admissions targets.
- Knowledge of hospice services, regulations, and the healthcare industry is a plus.
- Bilingual (English/Spanish) preferred due to the diverse Miami community.
- Valid driver’s license and reliable transportation required for travel within the area.
