Supported Living Coordinator 97
Schedule primarily Monday-Friday 8-4, but WILL require flexibility and afterhours and weekends. Must participate in rotating on call schedule.
SUMMARY: This position is responsible for the management and supervision of staff in the Supported Living Homes assigned to their caseload. It requires the successful candidate possess the ability to perform a wide variety of duties. The Coordinator will provide supervision of the Team Leaders and DSP's in the assigned homes and will provide training, mentoring and direction. Participate in the disciplinary actions of the staff supervised, complete annual evaluations and make recommendations for transfers, promotions, etc.
The overall goals of this position is to assist the Director in the daily operations of the Supported Living Department to train and supervise the Supported Living Team Leaders, and to ensure each home is well ran and provides superb living environments for the people in our care.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following and other duties may be assigned:
Coordinators will have a caseload of homes assigned to them.
Coordinators will know and keep up with all aspects of the homes, staff and individuals assigned. This includes, but is not limited to, getting to know people, spending quality time in the home, reading and learning support plans and ensuring individuals goals are being carried out, ensuring individuals are being cared for per policy and regulations as provided in training, and ensuring the homes are well maintained.
Quarterly, the Coordinator will complete the Quality Improvement Report for each home assigned. This report will require spending 2-3 hours in the homes when the individuals are present.
The coordinator will work a flexible schedule and participate in a rotating on-call schedule.
The Coordinator will train, supervise and mentor the Supported Living Team Leaders. The Coordinator may fill-in for the Team Leader when they are absent. This includes, but is not limited to, filling and/ or working open shifts in the assigned homes.
Do unannounced drop-in visits (at least 1 per week). In homes with 3 rd shift awake staff, 1 additional visit is required on 3rd shift per month. The 3rd shift visit must be a TRUE 3rd shift visit (between the hours of 2-4am).
Will complete, monitor and follow up with HR and training requirements for staff for the SL homes. The Coordinator will monitor each employee's orientation process and ensure they are thoroughly trained to work in the home.
Coordinators will monitor daily note$, Monitor for quality content-ensuring that questions of who, what, when and reactions or what was learned is accurately noted. This also includes ensuring data on the individuals' actions/ outcomes noted in their plan is documented in the daily notes.
Develop and monitor weekly schedules for persons receiving day services through the SL department. This includes monthly visits to all individuals who receive day services through the SL department, during their day service activities.
Monitor schedules for each site, this includes minimizing overtime. Be available to Team
Leaders as needed and assist Team Leaders with scheduling as needed. This includes encouraging Team Leaders to develop their own pool of substitute staff for the home.
Assist Team Leaders with shopping for food and supplies for the home, if there is a need and the Team Leader cannot feasibly or safely complete this task.
Communicating Daily with Team Leaders to ensure the needs of the home/ individuals are met and that schedules are maintained.
Monitor menus for all individuals and assist Team Leaders with the development of menus following dietary guidelines for each individual. Menus will follow the DIDD policies/guidelines.
Ensure all mealtime guidelines are followed. This includes being trained as a designated trainer for mealtime instructions for individuals on assigned caseload.
Monitor the assigned individual's health and be aware of all medical appointments, this may include attending critical medical and psychiatric appointments.
Be knowledgeable of and monitor individual's medications and more importantly medication changes. Obtain consents as needed for Psychotropic Medications and
changes timely to remain in compliance.
Coordinators must check the computer daily in the morning and afternoon for appointments for the individuals on their assigned caseload and alert the Team Leaders of any discovered or upcoming appointments.
Ensure that the MAR book accompanies each individual to their respective appointments and any forms that may need to be completed are available and completed at the appointment.
Human Rights consents/ Psychotropic medication consent forms must be signed BEFORE the first dose is administered. This includes any changes to already approved
medications.
Attend individual meetings as requested or necessary.
Be knowledgeable about all Supported Living individuals.
Assist in the development and opening of new homes
Assist Team Leaders in training staff for new homes.
Maintain and monitor Cost Plans, This includes ensuring any services needed are obtained.
Ensure progressive discipline policy and process is utilized when needed to correct and enhance job performance of employees.
Any other duties as assigned by the Director and/or Administration.

Qualifications
QUALIFICATIONS:
- Education and Experience:
Bachelor's Degree preferred. High school diploma required. Supervisory experience strongly preferred. 2 years of experience in the field required. Competent and efficient computer skills are required.
- Job Requirements:
Successful completion of the Medication Administration Course and a valid Driver's License with an F endorsement. Proof of current auto insurance coverage and personal vehicle must pass quarterly inspection. Must be able and willing to work and meet the demands of a flexible schedule and participate in oncall rotation, this includes working various shifts in the individual's homes.
