About the Role
We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at Orchard Brooke Assisted Living Center. As an LPN, you will play a vital role in promoting the health, wellness, and illness prevention of our residents.
As a member of our nursing team, you will work under the direction of a Registered Nurse and be responsible for administering treatment or medications, performing clinical procedures, and documenting patient data.
Responsibilities:
- Ensure the care and safety of all residents in the nursing unit
- Ensure that individual care plans are followed
- Administer and document medications, treatments, and other nursing services
- Makes a count of controlled substances on the assigned nursing unit
- Records current condition of assigned resident in EMR
- Receives and gives an accurate report of the resident condition at shift change
- Communicates to the nursing team and medical provider as indicated and required
- Assists in the orientation of new staff
- Maintains competency in all the tasks needed by CMS
Requirements:
- Minimum 18 years of age
- New York State Licensed Practical Nurse licensure (LPN)
- Strong written and verbal communication skills; able to follow written and verbal instructions
- Basic computer knowledge, PCC e-MAR experience preferred
- Physical endurance for frequent activity; ability to lift a minimum of 20 pounds
