Geriatric Medicine Nurse Practitioner Job Description
Overview
Our client in Indiana is seeking a Locum Nurse Practitioner preferably with Geriatric Medicine experience preferred.
Hours:
Full-time (Part-time considered); 6-month assignment (potential for extension)
On-Call Coverage:
- One week on, one week off
- Weekdays: 4:30 PM to 8:00 AM
- Weekends: Full day (Saturday & Sunday)
- Phone availability required, no travel.
Responsibilities:
- Provide primary medical management in collaboration with PACE Physicians.
- Conduct assessments, develop care plans, and provide treatment.
- Educate participants and caregivers.
- Act as part of the Interdisciplinary Team (IDT) and oversee the clinic.
- Perform rounds in contracted nursing homes and document participant changes.
Qualifications:
- Licensure: Active IN Nurse Practitioner license (required).
- Certifications: BLS, CDS, DEA, AANP/ANCC certification (required).
- Experience: 1 year with frail or elderly populations (preferred).
- Strong clinical, communication, and organizational skills.
- Proficiency in Microsoft Office Suite; familiarity with electronic medical records (TruChart transitioning to Epic).
Submission Requirements:
- Active IN license, BLS, CDS, DEA, and AANP/ANCC certification.
- 1 year of experience with geriatrics preferred.
