Summary/objective
Orchard Hospital and Orchard Hospital Medical Specialty Center are seeking a professional who works in collaboration with providers and other healthcare team members to provide medically related social and behavioral health services both in an inpatient and an outpatient setting. Under general supervision of a licensed clinician, this individual conducts social and behavioral health assessments and services in order for patients to attain or maintain the highest practicable physical, mental and psychosocial well-being. This individual works interdependently with medical providers to provide quality healthcare to our patients.
Essential functions
- Gather’s data of patient history through personal intake interviews
- Performs casework services with patients and relatives through individual assessments and treatment sessions
- Provides ongoing assessment, treatment planning and treatment of all patients assigned, under the supervision of a medical provider and licensed clinician
- Participates in multi-disciplinary team meetings to review patient treatment, concerns, progress and recommendations
- Supports and enhances the patient’s emotional stability and clearly explains the nature of the diagnosis and plan of care
- Maintains cooperative professional relationships with community agencies to ensure continuum of care
- Meets documentation requirements for billing and coding

- Demonstrates and ensures sustained department wide proficiency navigating through Orchard Hospital Health Information Services
- Read and respond to emails each shift
- Return phone messages within 24 hours

Work environment is a professional office setting with occasional events outdoors with moderate noise considerations.
Physical demands
Physical demands include frequent and occasional sitting, walking, standing, rotation, lifting, carrying, pushing, pulling, reaching, gripping, squatting, kneeling, balance, hearing, speaking and vision.
Travel required
Occasional travel to attend community or agency meetings and participate in community events.
Required education and experience
- Master’s degree in Social Work, Marriage Family Therapy or Counseling
- Must possess an intern number for licensure as either a Licensed Clinical Social Worker, Licensed Marriage Family Therapist or a Licensed Professional Clinical Counselor in the State of California
- Maintain a valid registration/intern number with the Board of Behavioral Sciences in the State of California
- Current health provider BLS card
Preferred education and experience
- Prior clinical experience as a paraprofessional is preferred
- Bilingual preferred
Additional eligibility requirements
Must possess current driver’s license and maintain liability or comprehensive auto insurance from a California carrier.
Work authorization requirements
Employee must also be able to satisfy the requirements of the Immigration Reform and Control Act of 1986, which requires documents to prove Employee’s identity and demonstrate that Employee is authorized to work in the U.S., and to complete an Employment Eligibility Verification form (Form I-9)
