Summary Of Duties
The Aging & People with Disabilities Vision: Oregon’s older adults, people with disabilities and their families experience person-centered services, supports and early interventions that are innovative and help maintain independence, promote safety, well-being, honor choice, respect cultural preferences and uphold dignity. The local offices determine eligibility and coordinate the delivery of these services within Umatilla and Morrow counties.
As a Human Services Case Manager, You Will
To qualify for this position, you must be bilingual in Spanish and English. A valid driver’s license and acceptable driving record is required.
A bachelor's degree in a Behavioral Science, Social Science, or a closely related field;
OR
A bachelor's degree in any field AND one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing);
OR
An associate degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing);
OR
Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

As a Human Services Case Manager, You Will
- Determine initial and on-going Medicaid eligibility for long term services and supports (LTSS).
- Develop and implement service plans.
- Coordinate the provisions of the service for in home, community-based care and nursing homes.
- Regularly review and update Medicaid eligibility and to monitor service plans.
- Coordinating the provision of services according to rules, regulations, and individual case plans.

Working Conditions
- Physical Exertion such as climbing stairs, walking on uneven terrain, and transporting a laptop or Bag.
- Working in a general office environment. Compose self in a Professional manner at all times.
- Must travel to individual’s homes, hospitals, nursing homes, and other provider settings as well as other offices and to Salem.
- May encounter environmental hazards including unsanitary conditions.
- May come in contact with people who present themselves as irrational, hostile, or angry who may become assaultive verbally, physically or with a weapon.
- May come in contact with animals, some of which may be dangerous.
- The individual in this position must be capable of driving a variety of vehicles which may be unfamiliar, drive to remote places in a variety of terrain, and possible in inclement weather.
- There may be exposure to a variety of contagious diseases such as staph infections, lice, MRSA, Covid-19, etc.
- The individual in this position will need to welcome constructive feedback and suggestions in an effort to strengthen work performance.
- Must maintain confidentiality in accordance with published guidelines.
- Be capable of managing a workload in a constantly changing work environment as well as plan for and resolve complex problems.
