What are the responsibilities and job description for the Clinical Supervisor Home Health care position at Origin Home Health Care?
The in-office RN /LPN is responsible for supporting the team and ensuring that patient care is coordinated and managed appropriately. This team member is responsible for ensuring that care and services are delivered appropriately as well as the supervision of clinical personnel.
RESPONSIBILITIES
Reviewing and transcribes patients' orders to ensure that we deliver the best care.
Monitor the office’s workflow and ensure that all processes are timely.
Reviews available patient information related to the case, including disciplines required, to determine home care needs.
Reviews patient’s clinical diagnosis, medications, procedures, and clinical course.
Assists clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing the plan of care.
Attends case conference meetings with organization personnel to facilitate coordination of care.
Complies with accepted professional standards and principles.
Consistently follows and uniformly enforces Agency policies and procedures to set an example for employees.
Assesses clinicians on an ongoing basis to ascertain their understanding and compliance with policies and procedures.
Participates in public relations and community activities that promote the Organization's role as an effective member of the health care delivery system.
Performs intermittent field visits on as needed basis.
Performs other duties and activities as delegated by the Director of Clinical Services.
QUALIFICATIONS
Licensed practical nurse with current licensure or registered Nurse with current license.
Must have recent Home Health Experience
Demonstrates excellent observation, organization, and verbal and written communication skills.
Proficient in Computers, Microsoft, Google Suite, EMR, and Technology
Knowledge of governmental regulations and CHAP standards or are willing to learn.
This position requires that the employee is willing and able to be a part of a busy team.
Job Type: Full-time
Pay: $52,000.00 - $79,000.00 per year
