Seeking a Weekend Admissions Liaison to preform pre-admissions screening and assessments on prospective patients / residents primarily for Holden Nursing Home and Oakdale Rehab & Skilled Nursing Center. This is a 100% remote position working 16-hour Saturday & Sunday from 8am-4 : 30pm with potential per diem coverage during the week.
RESPONSIBILITIES :
- Meets with customers and potential customers to communicate and market the company’s services.
- Develops and expands referral sources for both long term care and rehabilitation.
- Establishes and maintains relationships with other agencies and organizations within the community.
- Performs pre-admissions screening of prospective patients.
- Develops systems to ensure the budgeted patient census has been achieved on a monthly & quarterly basis.
- Secures and reviews medical information and reviews with appropriate departments.
- Verifies insurance / payment sources with business office.
- Attends required meetings to ensure appropriate resident placements are being made and to maintain an awareness of current status of programs and departments.
- Informs all required departments of potential admissions to facilitate a smooth and efficient process for the resident / patient.
- Schedules and coordinates admissions with all related departments.
- Plans and implements census development strategies.
- Monitors and limits spending to Department’s budget level. Provides recommendations as required.
- Conducts tours as needed and ensures appropriate other staff members have been trained in conducting tours properly.
- Performs all duties in a safe and efficient manner.
- Knowledge of JCAHO policies and practices
- Prepares tracking reports off all referral
- Participates in company sponsored marketing events (minimum of 2 per year)
