Key Responsibilities:

• Conduct In-Home Patient Assessments: Develop and implement individualized care plans that cater to each patient's unique needs and goals.
• Provide Top-Quality Nursing Care: Administer medications, manage chronic diseases, and provide wound care to ensure optimal patient outcomes.
• Educate & Empower Patients: Teach patients and their families about health management and self-care practices to promote independence and well-being.
• Collaborate with Healthcare Professionals: Work closely with physicians and other healthcare professionals to ensure coordinated and comprehensive care.
• Maintain Accurate Clinical Records: Document patient information and progress in compliance with regulations.

Requirements:

• Licensed RN: Current and unrestricted RN license in Massachusetts.
• Preferred Experience: At least 1-2 years of nursing experience, preferably in home health.
• Flexibility: Ability to work independently and adapt to varying patient needs and schedules.
• Strong Communication Skills: Excellent interpersonal skills to effectively interact with patients and their families.
• Tech-Savvy: Proficiency with electronic medical records (EMR) systems.
