What are the responsibilities and job description for the CCS Service Facilitator/Case Manager position at Orion Family Services?
Orion Family Services, an innovative outpatient and community mental health agency, is a growing organization that is looking to add a dedicated Service Facilitator to our team. We are a licensed outpatient mental health and AODA clinic. Our main office is located on the west side of Madison with satellite offices on the east side and in Monroe. Orion Family Services provides an array of outpatient, AODA and community-based services throughout southern Wisconsin. A Service Facilitator at Orion Family Services works directly with children and their families in Dane County who have been enrolled in the Dane County Comprehensive Community Services (CCS) program. They focus on helping clients achieve their goals through case management services. As a not-for-profit agency, our mission is to promote well-being through compassionate mental health and community services. Primary Job Responsibilities:
- Collaborate with program participant to assess strengths and needs and develop goals that aid in their recovery
- Coordinate services and resources that are relevant to the participants goals
- Lead a multidisciplinary team through collaboration and recovery team meetings
- Maintain timely and professional documentation such as assessments, recovery plans, and progress notes
- Support program participant through a trauma informed, strengths based, and client centered lens
- Regular travel within Dane County to meet with program participants within their home, school, and/or community
There may be opportunities to engage in other types of service delivery that Orion offers such as Individual Skill Development, Therapy (with appropriate credentials), group services, etc. More information can be provided during the interview process.
Qualifications
- Master’s Degree in a social work, human services, or related field
- OR bachelor’s degree in qualifying field and will be receiving master’s degree within six months of hire date
- OR bachelor’s degree with WI DSPS license as a Social Worker (#120)
- Experience providing case management services to children and their families with mental health needs
- Experience working within a multidisciplinary team
- Must pass WI Caregiver background check
- Valid driver’s license, insurance, and good driving record
- Professional written and verbal communication skills
- Ability to work independently and engage in ethical decision-making
- Understanding of confidentiality and HIPAA requirements
Benefits:
- Flexible Schedule: This position will start part-time hourly but will increase to a full-time salaried position within six months of hire as the caseload grows. Working hours are Monday-Friday, NO weekends.
