LMSW Clinical Social Worker-Dialysis
Orleans Community Health Medina Hospital is seeking a full time LMSW Clinical Social Worker to join our health care team at our Batavia and Medina Renal Dialysis Clinics.
Shift and Status
Full Time
Flexible scheduling if needed
Monday-Friday
No weekends, No holidays

Position Summary
The Medical social worker identifies, assesses and manages social and financial problems related to patient wellness, attainment of medical care, and health maintenance, providing social work services to patients and families in the Batavia and Medina Dialysis Clinics. This position must be able to recognize the psychosocial needs of patients, evaluate crisis situations and disability resulting from emotional, social and economic stresses of illness, counsel patients to deal with the particular stresses affecting them, participate in clinic planning and assist patients to understand, accept and follow medical and other professional recommendations to restore patients to optimum social and health adjustments; and arrange for specialized assistance from other sources within the clinic and from community resources.
Qualifications
LMSW
Master’s Degree in Social work program accredited by the Council on Social Work Education and the Department of Education.
Previous experience with counseling and case management in a nursing home, acute care setting or healthcare clinic.
