Responsibilities:
Job Summary
This position facilitates the appropriate, timely, and safe admission of new and returning patients. This is accomplished through coordination with hospital discharge planners and the in-house care team. This position also provides potential admissions, families, and referral services with Facility related information through Facility tours, personal visits, written materials, and verbal communication.
Duties
- Coordinate the admission process with the hospital.
- Promptly and efficiently responds to admission inquiries from hospitals, families, physicians, and other referral sources. This may include but is not limited to: taking admission phone calls, reviewing clinical and insurance information, giving tours, and managing bed availability.
- Alerts appropriate department heads and Facility staff of projected census changes including admissions, room changes, and discharges.
- Attends morning Daily Clinical Meeting to discuss census changes and potential admissions for the day.
- Maintains a close relationship with the DNS, Rehab Director, and Business Office Manager to maintain appropriate and efficient decisions about prospective admissions.
- Works closely with the DNS and housekeeping staff to coordinate safe room placement/bed selection for new admissions.
- Trains and coordinates appropriate staff to complete job duties as needed.
- Maintains a fundamental knowledge of federal and state regulations as well as reimbursement guidelines (Medicare, Medical, Insurance, etc.).
- Maintains a database of medical contacts and community resources.
- Observes resident rights and reports witnessed negligence/abuse to the state, the Facility Executive Director (ED), and the DNS per state regulations.
- Observe resident confidentiality.
- Follows the employee handbook rules and regulations as well as departmental and Facility wide policies and procedures.
- Other duties as assigned by the ED or DNS.
Knowledge, Skills and Abilities
- Demonstrates knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
- Requires mental maturity to deal with fast-paced environment and occasionally combative residents.
- Requires tolerance for body secretions, strong odors, exposure to diseases, and varied room temperatures.
- Demonstrated ability to communicate in English (reading, writing, speaking, and comprehension) in order to understand, plan, and carry out resident care plans and treatments.
