OrthoCarolina is currently seeking a Patient Navigator to work with our team at our OrthoCarolina Value Based Services Team in Charlotte. This position is a hybrid position coming on site to the Park Road location one (1) day a week and on-site for monthly meetings. The Patient Navigator provides a support service to OrthoCarolina patients with bundled care packages by providing a common thread that runs through the entire orthopedic or spine surgical journey. The Navigator helps tie together and streamline the comprehensive plan of care for each patient assigned. This role is to serve as a personal guide for the patient. The Navigator works through barriers that might otherwise impact the patient’s ability to receive quality care: physician appointments, facility scheduling, hospitalization, pre and post-surgical needs, medication, home health needs, finances, insurance and bundled payment plans, language translation, and travel facilitation, among others. Other responsibilities may include: Proactively plans out the services required for the patient so the experience is as pleasant and stress-free as possible. Works with the Test and Surgical Scheduler (TSS) to tighten the loop of communication so that every physician involved in a patient’s care is aware of the plan. Schedules all appointments for pre-op and post-op care: OC physician office, medical clearance/testing, patient education, hospital. Works with the patient and clinic scheduler to ensure surgery date and time meets the needs of the patient. Confirms pre-authorizations and medical records. Ensure medical clearance information is included in the overall patient care record. Helps patients control costs, providing resources as needed to help with the journey. Provides medical concierge service that contributes to exemplary customer service for our patients. Ensures that each patient and their care coaches are treated like VIPs, arrives at the right place at the right time, gets prescriptions and equipment as needed, facilitates post-op information and appointments and ensures key milestones are met. Maintains meticulous documentation of the patient’s needs, requirements and care plans. Relevant clinical experience considered. Three years of Orthopedic experience in clinic, patient education, patient advocacy, nursing, case management, social work, or similar experience. Maintain appropriate certification or licensure for the education held.
