Description
Responsible for the administration and management of the Adult Health Day Care. Supervise and coordinate work assignments for staff and ensure all program guidelines are followed. Responsible for planning, arranging, managing and coordinating appropriate health and social services for the maintenance and rehabilitation of Title 3E, OA3B, VA, Long Term Managed Care and Private Pay clients. Screen and triage clients for Adult Day Health Center.
1) Oversee and supervise all aspects of manage the Adult Day Care including staffing, daily activities and transportation of clients.
2) Supervise all basic services provided in Adult Day Care including self-care services oriented toward activities of daily living and personal hygiene, such as toileting, bathing and grooming.
3) Responsible for compliance with all licensure requirements for Adult Day Care as well as quality assurance.
4) Provide case management for clients who do not meet eligibility requirements for CCE, HCE or ADI programs for the Adult Day Health Center. Also, perform emergency case management as needed.
5) Evaluate the particular needs of each client on a quarterly and as needed basis and provide care and treatment, including medication supervision as indicated. Also, maintain narrative case management notes in client's medical records indicating the individual progress towards achieving health goals.
6) Provide short-term case management and coordination of funding for the Title 3B/3E program.
7) Provide back up for nurse, administer medication as prescribed by physicians following AHCA guidelines when needed.
8) Conduct client assessments to determine eligibility and appropriateness to attend the Adult Day Health Center under program guidelines.
9) Develop care plan in accordance with client needs and implement plan by providing, arranging and coordinating services.
10) Maintain contact with client according to clients needs and review care plan quarterly to assure appropriateness thereof.
11) Provide supervision of service provision, contact referral services and coordinate with other agencies to provide client with services not met by Council On Aging programs.
12) Follow all policies and procedures of the Osceola Council on Aging.
13) Other duties as assigned including assisting in times of disaster as needed.
Hours of work are 8:00 a.m. to 5:00 p.m. Flex time with the approval of your supervisor. This position may require some evening or weekend hours for special projects.
Requirements
Education/Training: Bachelor's degree in health care services, social work or a related field. A minimum of five years of staff supervisory experience and a minimum of three years of experience in providing services to persons diagnosed with dementia.
Must be a Registered Nurse and provide a current certificate of the license in the state of Florida.
Other requirements include a valid Florida driver's license and reliable vehicle. Also, ability to work in computer software programs including, but not limited to, Microsoft Office Suite. Must be able to read and write effectively.
Must have excellent interpersonal, communication and organizational skills as well as the ability to work with a wide variety of groups and individuals.
Fluency in both English and Spanish is preferred.
