Responsible for assigning, monitoring, and organizing cases. Directly involved in the lives of their clients, and provide resources and assistance. Coordinating services with community organizations. Case Manager must be highly organized, empathetic, compassionate, nonjudgmental, and eager to help vulnerable adults. 

ESSENTIAL FUNCTIONS:

  • Conduct client assessment and determine eligibility      under program guidelines.
  • Develop a case plan in accordance with client needs      and implement plan by providing, arranging and coordinating services.
  • Maintain contact with client according to client’s      needs and review plan periodically to ensure appropriateness thereof.
  • Provide case management for clients who do not meet      eligibility requirements for CCE or ADI programs for the Adult Day Health      Center. Also, perform emergency      case management as needed. 
  • Provide short-term case management and coordination of      funding for the Title 3E, Adult Daycare, in-facility respite program, and OA3B      Adult Daycare.
  • Coordination and liaison of services for the Adult Day      Health Center for the managed care long term care services programs, CCE,      and ADI programs.
  • Provide supervision of service provision, contact      referral services, and coordinate with other agencies to provide client      with services not met by OCOA programs. 
  • Compile and maintain progress reports related to      service provision and monitoring.
  • Follow all policies and procedures of Osceola Council      on Aging, Inc.
  • Other duties as assigned, to include assisting in      times of disaster as needed. 

OTHER RELATED FUNCTIONS:

Hours of Work: Full-time 40 hours/weekly- office open 8:00 am to 5:00 pm, flex time with supervisor’s approval. May require some evening or weekend hours for special projects. Additionally, the employee must be able to work off-site at any of the locations in the planning and service area. 

Requirements

Must be fluent in both oral and written Spanish and English 

Education/Training: Bachelor’s Degree in Social Work, Psychology, Counseling, or related field. Other related education or education may be substituted upon approval of the Area Agency on Aging. Requires the ability to follow verbal and written instructions. 

Licensure/Certification: N/A

Background: Must Pass Level 2 background/security screen. 

Experience: One to two years’ experience in Social Work, Social Services or Aging Programs is preferred. Must have knowledge of computer and word processing applications. Must possess valid Florida driver’s license and reliable vehicle.   

Interpersonal Skills: Must have excellent interpersonal, communication, and organizational skills and the ability to work with a wide variety of groups and individuals. Must be proficient in common English.

Essential Technical/Motor Skills Sufficient skills to be able to read and write effectively.  

Essential Mental Abilities: Knowledge and competent to perform all essential functions 

Essential Sensory Requirements: Ability to visually and audibly perform all essential functions. 

Essential Physical requirements: Must be able to sit for long periods of time. Sufficient mobility and strength to move about the facility and the community, have corrected vision and hearing to the normal range. This position may require occasional bending and lifting subject to specific physical requirements and working conditions as listed in job duties.
