- Provide comprehensive, patient-centered care to a diverse population, including infants, children, adults, and elderly patients.
- Diagnose, treat, and manage acute and chronic medical conditions, both independently and in collaboration with other healthcare providers.
- Perform routine physical exams, wellness check-ups, immunizations, preventive screenings, and health education.
- Manage and follow up on patients with chronic diseases such as diabetes, hypertension, asthma, and heart disease.
- Collaborate with a multi-disciplinary team of healthcare professionals.
- Maintain accurate and up-to-date patient records using the hospital’s electronic health record (EHR) system - EPIC.
- Educate patients and their families about health management, and preventive care
- Participate in community outreach programs to promote wellness and preventive healthcare.
- Adhere to hospital policies, procedures, and ethical standards.
- Medical degree (MD or DO) from an accredited institution.
- At least 1-2 years of experience working as a family medicine Provider
- Board Certification or eligibility in Family Medicine (or equivalent).
- Valid and unrestricted medical license to practice in Minnesota.
- CPR and ACLS certification (or willing to obtain).
- Previous experience in a family medicine or primary care setting, preferably in a rural or underserved community, is highly preferred.
- Strong clinical judgment and decision-making skills.
- Excellent communication skills, with the ability to connect with patients and their families.
- Ability to work effectively in a team environment and collaborate with other healthcare providers.
- Strong organizational skills and attention to detail.
- Flexibility and willingness to work in an unpredictable, resource-constrained setting.
- Experience with rural healthcare challenges and resource limitations.
- Proficiency with electronic health records (EHR) systems. EPIC experience a plus.
- Knowledge of telemedicine practices and technologies.
- Full-time position as a 1.0 FTE.
