What are the responsibilities and job description for the Advanced Practice Provider Cardiology Transitional Care position at OSF HealthCare?
POSITION SUMMARY:
This postion sits in our Transitional Care department. This department supports our cardiac program, specifically heart failure.
This APP position will focus mostly on inpatient rounding on all cardiac patients other than Cardiac surgery patients. This APP will work alongside a rounding cardiologist.
Will do new patient consults, discharges, promote continuity of care and help to get patients the correct follow up visits set up. Ideally seen in the transitional care clinic for their initial follow up visit and then set up with the cardiologist a few weeks later in office.
Schedule: Monday-Friday and potential for some hours on the weekends.
Will work closely with our APPs in transitional care to make sure our cardiac patients have all the resources they need while here as an inpatient and then upon discharge.

Qualifications
REQUIRED QUALIFICATIONS: Licensed to provide Advanced Practice Nurse Services or Physician Assistant Services Licensed by the Federal Drug Enforcement Administration and State of Illinois for prescribing of narcotics PREFERRED QUALIFICATIONS:
