What are the responsibilities and job description for the Advanced Practice Provider OnCall Connect position at OSF HealthCare?
POSITION SUMMARY: The Digital Health Advanced Practice Nurse (APN) is a member of a collaborative team who is responsible for providing digital health to a diverse population of patients. Utilizes evidenced based care and clinical decision support to guide practice. Performs ongoing communication and works to promote quality of care through collaboration with physician, advanced care providers and all service team members, patients and families. The APN demonstrates service excellence and accountability through recognizing the direct relationship between cost-effective and efficient provision of care and services including the impact of access to care. This diverse team enjoy a positive work environment that includes autonomous and collaborative care within the multidisciplinary team and a supportive work environment.
Qualifications
REQUIRED QUALIFICATIONS: Education: Masters Licenses/Certifications: APN-Advanced Practice RN Other skills/knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented. PREFERRED QUALIFICATIONS: 1 year of clinical experience
