Job Summary
We are seeking an experienced Home Health Nurse to join our team at OSF Home Care. As a Home Health Nurse, you will be responsible for providing skilled nursing care to patients in their homes, promoting independence and improving overall health outcomes.
Key Duties and Responsibilities
- Conduct comprehensive patient assessments to identify medical needs and develop individualized care plans.
- Provide wound care, including dressing changes and PICC line management, to patients in their homes.
- Collaborate with interdisciplinary teams, including providers, therapists, and social workers, to ensure seamless care coordination.
- Document patient information and progress using Epic, our electronic health record system.
Essential Skills and Qualifications
- A current State of Illinois RN license is mandatory.
- BLS certification is also a requirement.
- One year of experience as a Home Health RN is highly desirable.
- Proficiency in patient assessment, medication management, wound care, and IV therapy is crucial for success in this role.
Work Schedule and Benefits
Our Home Health Nurses work Monday through Friday from 8:00 am to 4:30 pm, with on-call responsibilities shared among staff members. Clinicians see an average of 5 patients per day and work every 4th weekend.
