**Job Responsibilities**
The home health registered nurse will be responsible for the following duties:
- Patient education on various topics, including disease management and wellness
- Conducting head-to-toe assessments to identify patients' physical and emotional needs
- Monitoring vital signs to ensure patients' overall health and well-being
- Performing skin and wound assessments to identify potential issues
- Providing dressing changes and wound care as needed
- Care for PICC lines, lab draws, foley catheter changes, and ostomy care
- Reconciling medications to ensure safe and effective treatment plans
- Demonstrating proficiency in Epic documentation
**Interdisciplinary Collaboration**
The home health registered nurse will work collaboratively with other healthcare professionals, including:
- Providers
- Therapy staff
- Social work
- Wound centers
- Infusions
**Required Skills and Experience**
To be successful in this role, the home health registered nurse should possess the following skills and experience:
- At least 1 year of experience as a home health RN
- Strong patient assessment skills
- Effective medication management and wound care techniques
- IV competency, including PICC, peripheral, etc.
- Lab draws and point-of-care testing (INR)
- Foley catheter and suprapubic catheter care
- Ostomy care
- Excellent communication and patient education skills
**Schedule and Work Environment**
The home health registered nurse will work an average of 5 patients per day, Monday through Friday, from 8:00 am to 4:30 pm. On-call weeknights and weekends are required on a rotation basis, with regular day shift work every fourth weekend.
**Staff Development and Training**
New employees will participate in comprehensive training, including:
- 1:1 training sessions
- Wound care and IV class training
- Training on the Canvas platform and Health Stream online modules
- General orientation
Approximately 5-7 days are required for this initial training process.
