Job Description

Job Title : Registered Nurse (RN)

Job Specialty : Home Care

Job Duration : 13 weeks

Shift : 8-hour days, Monday - Friday, with rotating on-call duties weeknights and weekends every 4th weekend

Guaranteed Hours : 40 hours per week (except during orientation)

Experience : At least 1 year as a Home Health RN

License : State of Illinois RN license

Certifications : Basic Life Support (BLS)

Must-Have :

Patient assessment skills
Medication management
Wound care
IV competency (PICC, peripheral, etc.)
Ability to communicate effectively
Patient and family education skills

Job Description :

Provide patient education and conduct comprehensive assessments including vital signs, breath sounds, and skin / wound evaluations

Perform dressing changes, wound care, management of Wound Vac, and PICC line care

Execute lab draws, foley catheter changes, and ostomy care

Conduct medication reconciliation and patient education

Collaborate with providers and interdisciplinary care teams including therapy staff, social work, wound centers, and infusion providers

Utilize equipment such as laptops, portable printers, and cell phones for effective communication and documentation

Use Epic for documentation purposes

See an average of 5 patients per day and ensure thorough and timely documentation

Participate in 1 : 1 staff development, wound care classes, IV training, and online modules through platforms like Canvas and Health Stream during orientation

Dress Code : Professional scrubs

Orientation and Training : 5-7 days including general orientation and specific training activities
