Description

Title:                            Youth and Family Services Case Manager

Dept/Program:            New Beginnings/Homeless Youth Services

Reports to:                  Senior Program Manager

Status:                         Full-time, Non-exempt

Position Overview:
Primary point of contact for youth and families experiencing homelessness. Provides trauma-informed intake and case management using positive youth development and strength-based adult empowerment frameworks. Focuses on building client capabilities and self-sufficiency while supporting transition to stable housing.

Essential Duties:

Assessment & Planning:

  • Process Coordinated Entry referrals and determine program eligibility
  • Conduct strength-based assessments to identify goals and needs
  • Create and regularly update individualized service plans
  • Support housing search and placement process
  • Review and update benefit eligibility
  • Develop sustainable exit plans with ongoing supports
  • Other Duties as assigned

Direct Client Support:

  • Provide trauma-informed crisis intervention and case management
  • Conduct regular client check-ins and progress reviews
  • Connect clients to resources through warm handoffs and follow-up
  • Build self-advocacy skills through coaching and skill development
  • Maintain relationships with property managers and landlords
  • Participate in case conferences and team meetings
  • Gather and incorporate client feedback
  • Other Duties as assigned

Administration:

  • Maintain detailed case notes and documentation
  • Update client databases and tracking systems
  • Complete required program reports and paperwork
  • Track outcomes for grant reporting
  • Maintain community resource contacts
  • Participate in professional development and training
  • Collaborate with internal/external service teams
  • Other Duties as assigned

Qualifications


Qualifications:

  • Associate degree in human services or equivalent experience working with youth and families or
  • Two years relevant experience in case management, youth services, or homeless services
  • Valid Arizona driver's license and reliable transportation for regular client visits and community meetings
  • Current auto insurance with clean driving record (verified annually)
  • COVID-19 vaccination with up-to-date boosters as recommended
  • Ability to pass extensive background check and maintain fingerprint clearance
  • Clear TB test with willingness to update as required
  • Minimum age: 21 years

Skills and Knowledge:

  • Understanding of trauma-informed care principles and ability to apply them in daily interactions
  • Crisis intervention and de-escalation techniques with focus on client and staff safety
  • Cultural competency and demonstrated ability to work effectively with diverse populations
  • Professional boundaries maintenance while building trusting client relationships
  • Microsoft Office proficiency including Word, Excel, Outlook, and PowerPoint
  • Database management experience with attention to accuracy and timeliness
  • Strong case documentation skills that balance detail and efficiency

Schedule:

  • Regular weekday hours with occasional early morning or evening flexibility
  • Some weekend availability for client emergencies or special programs
  • Ability to adjust schedule to accommodate client needs and crisis situations

Compensation:

Salary for this position starts from a base of $18.00/hr.

Physical Demands:

While performing the duties of this job, physical demands the employee will regularly encounter include sitting, standing, climbing stairs, walking, driving, carrying up to 20 pounds, operating a computer keyboard, firm or strong grasping and repetitive hand control, making and receiving telephone calls. The job requires the ability to work in stressful situations from time to time and remain focused for extended periods of time. 

Work Environment:

The worker is subject to both environmental conditions, working inside and outside.
