Our Hospice is looking for Home Health Aides (HHA) for our home side patients. This person works with a caseload of patients alongside a RN Case Manager. Our patients and families are in the most important time of their life and need non-skilled nursing to help navigate their pathway. In this position, every day is a little different and you are on the go visiting patients in your community. This role will work Monday-Friday, no holidays or weekends. The hours are 8 am – 4:30 pm, with some flexibility.

An HHA at Our Hospice Will:
- Provide routine patient care in the patient's home or long term care facility per the Home Health Aide assignment in an exemplary manner, providing comfort, care, and compassion to patients and families.
- Assist the patient with daily hygiene activities (including, but not limited to: helping patients to shower, change clothing, shave, and toilet).
- Observe patients and reports observations and concerns to the primary care nurse and/or Patient Care Coordinator, Nurse Facilitator or Manager of Inpatient and Bartholomew.
- Complete patient records accurately in Home Care Home Base (HCHB) and on a timely basis.
- Assist in maintaining a clean, healthful environment for the patient.
- Attend and participates in Interdisciplinary Group meetings, in-services, and staff meetings as requested.
- Participate in a minimum of twelve (12) hours in-service education per year.
- Be willing and able to lift patients for routine hygiene as needed
- High school diploma or general education degree (GED)
- Licensed Home Health Aide (HHA) or Certified Nurse Aide (CNA) in the State of Indiana
- Interest and ability in providing patient care in homes and facilities
- Ability to travel via your own vehicle
- Valid Indiana Driver's License and car insurance
