A Patient Care Manager at Our Hospice is responsible for coordinating and ensuring delivery of high quality of care to patients and for maintaining high collaboration with referral sources and community partners. Assures continuity, cost effectiveness, and quality of the operations, and provides leadership to the clinical staff and other areas of Hospice. Maintains knowledge and compliance with organizational policies, along with state and federal regulations pertaining to hospice care. Assures utilization of performance improvement principles and practices. Interacts with other leaders and patient care managers and with support departments to achieve office and organizational goals.
A Patient Care Manager will:
Serve as a role model for professionalism and teamwork within the hospice staff.
- Lead by promoting established Standards of Service and demonstrate expertise in hospice and palliative care.

Support hospice leadership and adapt to organizational changes.
Manage priorities effectively and collaborate with support departments.
Participate in Clinical Leadership Team meetings, advising on compliance and clinical issues.
Report compliance and HIPAA concerns appropriately and address staff performance issues.
Encourage volunteerism and staff participation in hospice activities.
Act as a clinical resource, ensuring quality care throughout the patient journey.
Coordinate services to maintain cost-effective healthcare delivery.
Collaborate with interdisciplinary teams to assess and meet patient needs.
Ensure proper documentation for billing and oversee clinical excellence.
Teach and supervise staff, providing ongoing training and support.
Utilize a participative management style, valuing staff feedback and keeping them informed of changes.
Maintain proficiency in electronic medical records to enhance patient care.
Promote continuous quality improvement, respond to complaints, and ensure compliance with regulations.
Involve staff in quality initiatives and review performance metrics regularly.
Assist in implementing organizational strategies and reviewing success metrics.
Collaborate on educational needs, participate in hiring and onboarding, and promote staff engagement and retention.
Address staff satisfaction and recognize achievements.
Foster an equitable work environment and leverage diversity for effective teamwork.
An Quality Specialist at Our Hospice Needs:
- Bachelor of Science in Nursing Degree required
- Three to five years' experience in hospice preferred
- Demonstrated experience in managing patient care teams and managing a 24/7 nursing unit preferred.
- Valid Indiana Registered Nurse License
- Valid Indiana Driver's License
- Leadership and managerial skills
- Knowledge of healthcare regulations
- Ability to read, analyze, and interpret general industry periodicals, professional journals, technical procedures, or governmental regulations
- Ability to effectively present information and respond to questions from staff, clients, and customers
- Ability to apply concepts such as fractions, percentages, rations, and proportions to practical situations
- Excellent problem solving ability
- Utilize Microsoft Word, Excel and internet
