Job Requirements
BLS certification required
Nursys license required within 30 days of start
Primary Source Verification of professional license required within 30 days of start
NRP certification required
FMOLHS Skills Checklist required within one year prior to start
FMOLHS Competency Exam required with passing score of 80% or higher
FMOLHS Core Competencies required with passing score of 80% or higher
MMR immunization proof required
Fit Test required within one year prior to start
Hepatitis B immunization proof required
Influenza Vaccine proof required annually
TB Test proof required within one year prior to start
Health Statement required within one year prior to start
Varicella immunization proof required
Tdap immunization proof required
Annual TB Questionnaire required within 6 months of start
