Certification Details BLS State Licensure Job Details On call one night per week and rotating weekend call Must have pediatric experience. Job Requirements Must have pediatric experience. Additional Details Proof of 2 immunizations, positive titer or Declination accepted. Must be one of the 3 mask fit types. N95- 3M 1860, 1860s or 1870. Must be completed within 1 year prior to start. Updated annually. Proof of 3 immunizations, positive titer or Declination accepted. Flu Season dates to be communicated each year. Only Medical / Religious exemptions will be accepted with a physician’s signature or written signed statement from traveler. Proof of negative Blood test or Skin test as described below. Statement signed and dated within 1 year of start stating traveler is able to work without restrictions. Proof of vaccination within past 10 years or Declination accepted. See document library.
