Job Requirements
- Proof of 2 immunizations, positive titer or Declination accepted for MMR.
- Must be one of the 3 mask fit types. N95- 3M 1860, 1860s or 1870 . Must be completed within 1 year prior to start. Updated annually.
- Proof of 3 immunizations, positive titer or Declination accepted for Hepatitis B.
- Flu Season dates to be communicated each year. Only Medical / Religious exemptions will be accepted with a physician’s signature or written signed statement from traveler.
- Proof of negative Blood test or Skin test as described for TB Test.
- Statement signed and dated within 1 year of start stating traveler is able to work without restrictions.
- Proof of 2 immunizations, positive titer or Declination accepted for Varicella.
- Proof of vaccination within past 10 years or Declination accepted for Tdap.
- See document library. Required within 6 months of start for Annual TB Questionnaire.
- Resume or Profile required.
- 2 references required; 1 Charge or Higher; 1 must be supervisor reference.
- Must be completed within 30 days prior to start for background checks.
- If not uploaded with background check. To be complete within 6 months of start updated annually for Social Security Trace.
- Verification required within 30 days prior to start date for OIG Search.
- Included in background or run within 30 days of start, updated annually for EPLS.
- Verification required within 30 days prior to start date for National Sex Offender.
- Negative 10 Panel within 30 days prior to start for Drug Screen.
- Must be Ishihara Color Test within 1 year prior to start.
