ob Summary:
Reporting directly to the SR. Vice President of Northern Operations, the Qualified Intellectual Disabilities Professional (QIDP) is responsible for participating as a member of the facility team to ensure safe and effective residential services. The QIDP oversees all aspects of client care and assists with training support personnel. The QIDP at Progressive Housing, Inc. is a positive role model, adaptable, mission oriented, goal driven, highly organized, efficient, and exercises good judgment and decision making skills.
ROTATING WEEKENDS AND ON CALL HOURS ARE REQUIRED FOR THIS POSITION
Job Duties and Responsibilities:
The statements below describe the general nature and level of work being performed in this role. They are not intended to be a complete list of all duties and additional responsibilities may be delegated as required:
1) Ensure that CILAs are in compliance with current policies and procedures, state and federal rules and regulations, and assist with ongoing development, reviews and revisions
2) Provide feedback and support to direct care staff (DSP’s) in conjunction with their supervisor (the House Manager), while maintaining positive working relationship. This includes, but is not limited to new employee orientation.
3) Attend Person Centered Plan (PCP) meetings that are organized by the ISC.
4) Develop and implement the implementation strategy plan that coincides with the PCP.
5) Ensure appropriate implementation of outcomes for the residents on your caseload.
6) Follow up with Implementation Strategy Plans (ISP) and ensure that staff is properly executing the outcomes assigned.
7) Review individuals’ needs with community support team which includes the BCBA, RN, and House Manager to ensure the individuals on your caseload are receiving appropriate services (annual and six month reviews).
8) Assist or attend House meetings along with House Manager to ensure any concerns or issues pertaining to the individuals on your caseload. Submit completed in-service training documentation to supervisor at the end of the training.
9) Thoroughly complete quarterly environmental reviews and reports.
10) Complete monthly census for each client of caseload. Monthly census is due by the 1stof every month.
11) Complete monthly progress notes by the 15th of the following month, monitor clients, and review all required documentation to ensure continuity of all clients’ care and programming needs.
12) Ensure that all files and reports are filed promptly in the client’s record.
13) Complete onsite day training visits at least bi-weekly and more often if necessary or as directed.
14) Monitor clients in their residential setting, at least weekly, to ensure appropriate implementation of programs and outcomes.
15) During residential visit observe for appropriate dress and hygiene prior to leaving the house for day training and appointments.
16) Review all client records on a quarterly basis to ensure that required documentation is completed timely, thoroughly, accurately, and if any changes are necessary
17) Attend monthly staff meetings, weekly management meetings, agency in-services, and other meetings as directed by supervisors
18) Attend staffing, house meetings, safety, infection control, treatment review, human rights, behavior management committee and clinical monitoring.
19) Schedule and chair all intake and discharge staffing, special team meetings and any other periodic reviews for client on your caseload
20) Handle all complaints from individuals, guardians, agencies and families. Direct to the supervisor if unable to come to a resolution.
21) Monitor Therap and that Communication Logs (S -Comms and T Logs) and ensure they are being completed and are current
22) Ensure that all clients are offered opportunities for community integration and that all clients’ rights are upheld and treated with dignity and respect.
23) Always place the safety and welfare of the clients first.
24) Complete projects and special assignments by establishing objectives, determining priorities; managing time; gaining cooperation of others; monitoring progress; problem-solving; making adjustments to plans.
25) Review all clients’ status quarterly to determine if any changes are necessary in programming, supports, or environment.
26) Ensure all DSPs abide by and endeavor to fulfill the corporate mission.
27) Set a positive example for all partners in regards to work ethic, mode of dress, personal demeanor and teamwork. Follow dress code as directed by the facility.
28) Ensure that all resident rights are upheld and all clients are treated with dignity and respect.
29) Report all complaints or grievances made by residents, employees, or visitors.
30) Sign and date all entries you make in the resident’s clinical record.
31) Report all occupational injuries immediately.
32) Required to go to the hospital to assist with the hand off of services and to ensure the rights of the individual are met.
33) Be on-call for any incidents, injury, or behavioral concerns.
34) Provide detailed written summary/follow ups to the IDT regarding all medical/behavioral incidents and/or hospitalizations.
35) Complete annual redeterminations for assigned clients to ensure continuance of state benefits.
36) Communicate individual client status with Finance and Behavior Department for billing purposes.
37) Other miscellaneous duties as assigned
Qualifications:
Education and Licensure:
1. Must meet State educational requirements for QIDP (Bachelor’s degree in a human service related field)
2. Must become an approved QIDP and DSP trainer with the State of Illinois
3. QIDP Certification preferred; QIDP experience required
Other Requirements:
1) Must have valid driver’s license and comply with company policy for driving vehicles

Physical Demands:
1. Must be in good health and free of communicable disease.
2. Moderate to heavy physical exertion required to perform job duties. Duties will require reaching above and below the waist, bending, stooping, lifting up to 50 pounds, stretching, walking and standing, which may occur on smooth, rough, or uneven surfaces.
Mental, Visual and Aural Demands:
1. Must have the mental ability to understand and perform the requirements of the duties.
2. Must have functional hearing and vision.
3. Must be able to read, write and understand written English communication.
4. Constant mental, visual and aural attention necessary to perform duties satisfactorily.
5. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, agitated, confused and sometimes hostile clients within the facility.
Safety – Self and Others:
1. Is familiar with and practices good body mechanics to prevent injury.
2. Be knowledgeable of standard precautions and practice good infection control techniques.
3. Is familiar with the placement and use of emergency equipment and procedures to follow in case of emergency or natural disaster.
4. Be knowledgeable of emergency behavior management techniques as set forth by the Crisis Prevention Institute (CPI).
Work Environment:
1. Work may be indoors in a clean, well-lighted, well-equipped house or in a community setting.
2. May be subjected to multiple distractions and interruptions while working.
3. Subject to possibility of infection from communicable diseases, slips, falls, physical and verbal abuse from residents and possible muscle strains / sprains from lifting.
