The work of a Direct Support Worker is directed by a plan of care developed by the Oregon Department of Human Services.
The direct support worker follows the individual service plan for each individual.
Client 1 :
- Male 16 yrs old Eugene 97402
- Desired Schedule : Mon-Fri 3 : 30pm-6 : 30pm
- Likes : schooling, incredibly intelligent, he loves his Mt.
Dew, technology, video games, watching videos games, social media, tech savvy and likes computers / in dept technology.
His online community and friends, Sports of all kinds, loves to be active, especially swimming, basketball, languages.
He is learning German, Spanish and Russian
Needs : a provide to be aware of his needs and provide support as needed during those times.
Help him regulate emotions, and physical conditions / needs.
Model language and communication, help to communicate better.
Patiently reinforce boundaries and socially appropriate behavior.
Help client to initiate and complete tasks as independently as he is able.
Work on his goals and ADLS together.
Client 2 :
- Male 14yrs old Eugene 97402
- Desired Schedule : Mon Fri 3 : 30pm- 9 : 30pm (sometimes until 10 : 00pm)
- Likes : Thomas the train and McQueen, playing on iPad, going to baseball games, enjoys bowling, very smart, good reader, loves to laugh, have fun, and do physical activities like running and riding bikes.
- Needs : Direct instructions that are short and basic.
Instructions one at a time work best, offer him choices, social stories, applicant must be familiar with navigating behaviors, work on increasing his ability to complete chores at home, and working on his goals and ADLS
These clients are siblings and it would be great if the DSP is able to work with both clients.
Not at the same time- but a rotation if ever needed)
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
Other duties may be assigned.
- Deliver training, support, and assistance to the clients in the areas of personal care, recreation and leisure, daily living skills, social skills, health needs, personal preferences, exercising of individual rights, community integration, and behavior management.
- Participate in the implementation and monitoring of the clients individualized support plan.
Your observations and opinions are crucial to tailoring an achievable plan.
- Accurately complete required documentation including but not limited to clocking in and out, weekly progress notes, and communication with the family of the client and the office staff.
- Ensure clients health and safety at all times including but not limited to prevention of abuse, perform CPR, perform Standard First Aid, and other needs that the client may stand in need of.
- Work directly with the client / family of the client to ensure the goals of the client are being met and being worked towards
- Cooking, cleaning, medication assistance, scheduling, attend medical appointments, shopping, and more...
- Skill development and support is defined in the individual’s Individual service plan
- Documentation of progress notes with each shift
Qualifications and Education Requirements :
- High school diploma or equivalent
- Personal Support : 1 year previous experience (required)
- Transporting individuals
- Be certified in CPR and First Aid prior to clocking a shift
- Overall competency in working under pressure
- Experience in providing general care and support
- Knowledge of home care services
- Proficient in written communication
- Proficient in oral communication
- Be at least 18 years of age
- Understand requirements of maintaining confidentiality and safeguarding individual information.
- Be able to communicate with individuals, health care providers, case managers, and appropriate others.
- Be able to respond to emergency situations at all times services are being delivered.
- Must have a valid Drivers License, Auto Insurance, and reliable transportation in compliance with the laws of the Department of Motor Vehicles.
