Summary:
Member of Outreach Team responsible for intensive case management of families with children in out-of-home placement with the goal of transitioning to permanent placement with the parent.
Duties
· Acts as primary liaison with Office of Youth & Family Services (OYFS) caseworker(s); collaborates in development of transition plans and placement/care recommendations.
· Supports, coaches, case manages and monitors children and their families/caretakers from the referral phase through reunification and in some cases beyond. Provides supervised parent/child visits where appropriate. Provides information and referrals; and secures supportive services and resources. Travel to and from children in placement, their family members or other appointments is often required. Some evening & weekend hours required.
· In collaboration with OYFS, continuously assesses goals and service needs of children and families/caregivers identified for intensive case management services before and after reunification.
· Cultivates community resources for children and families/caregivers with complex needs. Coordinates with OYFS, Outreach’s team, outside counselors and other service providers in order to address the challenges of reunification and to resolve service gaps.
· Attends OYFS Family Team Conferences, Monthly Staffing and other meetings as required
· Prepares and maintains case records, reports and other appropriate documentation; enters client data into agency data collection system
· Performs other duties as assigned.
Qualifications:

Education/Experience:
Master’s or Bachelor's degree in Counseling, Psychology, or Human Service and related experience required.
Computer Skills:
To perform this job successfully, an individual should have proficiency using Microsoft Word processing software; Microsoft Excel spreadsheet software; Internet software and Database software.
Certificates and Licenses:
Crisis Prevention and Intervention training preferred; Social service/MH/BH and case management experience preferred; Regional travel at various times/days to visit and/or transport children in placement and their families; Reliable transportation and valid PA Driver's License & auto insurance is required. Spanish language proficiency is preferred.
Work Environment:
The noise level in the work environment is usually moderate.
Physical Demands:
The employee must frequently lift and/or move up to 20-25 pounds.
