Position Announcement Registered Nurse
Outside In seeking a dedicated and passionate Registered Nurse to help provide care to the underserved community including folx at all ages and stages from across the spectrum of lived experiences and needs.
This Registered Nurse Opportunity
The Registered Nurse (RN) provides care management of primary care client populations with chronic illness and/or multiple co-morbidities for the primary care team at the Main Building. This position works in coordination with multiple primary care providers, behavioral health providers, and care team members. Through skilled assessment, education, collaboration, and coordination of healthcare and community resources, this position helps patients achieve optimum functional health status and improve quality of life. This position also provides triage in-person or by phone as needed.
This is a full-time, exempt, union-represented position with a starting annual pay of $90,104.35 to $99,458.35, depending on skills and experience. Work in collaboration with multidisciplinary providers and clinic team members to provide innovative, patient centered, evidence-based care.

Actively care manage both acute conditions and chronic care clients.

Provides resources and patient education for initial prenatal intake and then coordinates with primary care provider for first prenatal appointment.

Assist with triage or nurse advice of urgent matters that occur over the phone and in person.

Provide general guidance and education on lactation activities and services for postpartum and newborn visits.

Demonstrate effective listening, motivational interviewing, harm reduction methods, and narrative skills.

Certified Lactation Counselor or Certified Breastfeeding Counselor Certification highly desirable or must be willing to obtain one or have one-year previous experience in mother baby.
Preferred Qualifications

Bilingual Spanish & English highly desirable.

Experience with chronic disease management and high-risk populations highly desirable.

Experience with providing pediatric or primary care with this population is highly desirable.

Experience with prenatal, mother baby, or lactation care highly desirable.
