POSITION DUTIES/ESSENTIAL FUNCTIONS:
Assist patients with bathing, toileting, dressing, shampooing, feeding, shaving, mouth care, etc.
Assist with simple exercise programs, application of prostheses and braces, ambulation and transfer activities (including use of assistive devices) and accompanying patient out of doors.
Assist with medications normally self-administered or remind patient as directed by the primary care nurse.
Planning, shopping and preparing meals accordingly to prescribed diet.
Light housekeeping activities, such as changing bed linens, laundry and cleaning as necessary to maintain a healthy environment.
Accompany patients to the physician or clinic, etc., when part of the care plan and approved by direct supervisor.
POSITION REQUIREMENTS:
High school diploma or equivalent - required
Valid Home Health Aide Certificate - required
Current MA Driver’s License and reliable transportation
Successful completion of the Home Health Aide Competency Exam with a minimum overall score of 80% and minimum 80% in each section - required
