POSITION SUMMARY :
The Registered Nurse Case Manager plans and provides individualized nursing care and comprehensive services to home care patients. The Registered Nurse Case Manager assists in clinics and other agency programs as assigned by the Nursing Manager. The Registered Nurse Case Manager is expected to meet established productivity standards (to include weekends / holidays as needed) as well as a rotation of on-call responsibilities.
PRINCIPLE DUTIES / ESSENTIAL FUNCTIONS :
- Assesses patient’s health problems and needs and develops plan of care.
- Provides direct skilled nursing care as per physician orders.
- Observes patients, charting and reporting changes in patient’s conditions, such as adverse reactions to medications or treatment and taking any necessary action.
- Document in medical record and prepares other reports as needed.
- Educates patient and families in self-care and health management.
- Collaborates with the physician and other home health care providers, within and outside agency to enhance patient care.
- Provides case management when appropriate.
- Attends case conference as requested.
- Adheres to policies and procedures.
- Participates in agency quality management and safety activities.
- Must comply with organizational vaccine mandates and maintain required licensure and certification.
