This position is responsible for supporting people receiving services to be involved and included in their communities, helping them to learn applicable life skills, assisting with personal needs, and facilitating relationships with others. Minimum Requirements:
- High School Diploma (required for Nevada employees, preferred but not required for all other locations)
- Must be able to read, understand and communicate information and ideas verbally and in writing so others will understand.
- One year experience in the human service field or equivalent. Must have a background in working or advocating for individuals with developmental disabilities.
- Current CPR and First Aid certification (or ability to (re)certify within 30 days of employment).
- Must be able to lift and transfer up to 50 lbs.
- Healthy mental and physical state to remain attentive and alert to ensure the health and safety of the people we serve.
- Physical ability to stand, walk, use hands and fingers, reach, stoop, kneel, crouch, talk, hear, and see during work hours, especially when working directly with the people we serve.
- Must have a minimum of 3 years of driving experience, ability to use route navigation apps, and general knowledge of the area preferred.
- Must have reliable transportation to drive participants to appointments or other activities if company vehicle is not available.
- Must possess a valid Driver’s License from the state in which you reside, maintain a clean driving record, and proof of insurance per company insurance guidelines.
- Must pass a motor vehicle record (MVR), background check, and other screening per Company requirements.
Essential Job Functions (Included, but not limited to):
- Provides integrated support to the individual with developmental disabilities.
- Participate in the implementation of the individual Service and Support Plan by attending regularly held staff meetings, implementing concepts learned during meetings, trainings, and following written support goals.
- Document daily on the person’s habilitative progress towards their community access goal.
- Be the first line of communication while talking with guardians and Host Home Providers (HHPs) about the care and wellbeing of each person in service.
- Be on time, never late, when picking up and dropping off a person in service.
- Maintain confidentiality at all times while out in the community and at base site. Exercise discretion when sharing information with other staff members or when in the presence of other people receiving services.
- Respect the rights of the person in services and support them to know their rights.
- Ensure that service is delivered in a caring and respectful manner, in accordance with relevant agency policies and industry standards.
- Keep direct supervisor informed of problems and concerns related to individual customers, families and/or operations of the program/service and write incident reports, as appropriate, in accordance with company standards established to ensure reporting within state-required timelines.
- Maintain standards where rights are suspended and provide appropriate reports to the Human Rights Committee when necessary.

Physical Requirements - Requires repetitive manual and finger dexterity and eye- hand coordination; requires the ability to use all office equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics.
- Visual, Hearing, and Communication Requirements - Must be able to read, understand and communicate information and ideas orally and in writing to effectively communicate with others.
Pressure Factor- This position may require employees to adapt to a frequently changing environment, including handling and de-escalating incidents. This position will interact with individuals who are intellectually and developmentally delayed, may be frail, confused, and at times engage in verbal or physical aggression. Accordingly, it is sometimes necessary to deal with unpleasant situations with the need to problem solve.
