What are the responsibilities and job description for the Case Manager position at OVP Health?
The case manager coordinates and monitors the assessment, treatment planning process and follow-up for each resident, and serves as an advocate and liaison for and on behalf of residents’ rights and benefits.
PRINCIPAL DUTIES AND RESPONSIBILITIES:

· Participates in client pre-placement planning. Conducts the orientation and initial, quarterly and annual assessment of individual client needs.
· Ensures that all participants complete the individual plan for contracted mental health services.
· Coordinates and chairs the interdisciplinary treatment planning meeting and writes the individual plan of service.
· Facilitates community resource availability based on client needs (e.g., day program, guardians) to ensure maximum social integration.
· Evaluates, facilitates and monitors services and clients’ progress in relationship to established goals and objectives. Documents monthly in the medical record.
· Provides monthly monitoring overview and ensures that all services have been delivered and properly accounted for before submitting billings to fiscal office. Evaluates and signs approval of the monthly home occupancy report and invoice.
· Evaluates the monthly personal care service log.
· Evaluates and approves the personal allowance fund record.
· Facilitates client benefits (e.g., SSI, Medicaid, SSB, Medicare).
· Reviews and coordinates with the team and provider on decisions involving hospitalization, respite and transfers/level-of-care changes. Implements plan of action. Maintains ongoing dialogue with intake regarding status changes.
· Completes discharge note in the medical record on planned discharges. Completes comprehensive discharge summaries on planned/unplanned discharges within 14 days.
· Provides crisis intervention for assigned caseload.
· Maintains assigned medical records as required by medical record procedures.
QUALIFICATIONS:
- Bachelor's degree in social work or human services field.
- 1 year work experience or Master's Degree in Social Work.
- Two to three years of experience working in community mental health.
- State social worker registration or equivalent license to be approved for eligibility as a qualified mental health professional.

POSITION TYPE/EXPECTED HOURS OF WORK:
This is a full-time position. Hours are scheduled to adequately cover a 24/7 inpatient facility.
