- Job Responsibilities
- Assessment of complete chart and knowledge of patient's medical condition, financial status, emotional needsand support systems.
- Collaborates with the Medical Director, Director, Manager, and Supervisor of Case Management on patientswho are not meeting the regulatory requirements for hospitalization.
- Provides initial and concurrent reviews for approval of hospital stay for all patients. Notifies patients whenstay has been denied.
- Collaborates with the interdisciplanary team to ensure patient quality of care and safety of discharge.

Responsible for the coordination and facilitation of the plan of care.
Associate's degree in Nursing or Diploma in Nursing required. Bachelor's degree in Nursing preferred.
Licensed as a Registered Nurse (RN) required. Eligible to practice nursing in the State of Kentucky required.Certified as a Basic Life Support (BLS) Healthcare Provider preferred.
2 to 8 years progressive experience in a related field required. Experience in Case Management preferred.
Requires critical thinking skills and decisive judgment. Works under minimal supervision. Must be able to workin a stressful environment and take appropriate action.
