Case Manager
FLSA Status: Non-Exempt Reports to: Program Director
Job Summary:
The Oxbow Center Case Manager plays a crucial role in client advocacy; ensuring clients receive the necessary care and services to improve their quality of life.
Essential Functions:
Clinical Duties
- May provide specialized services or treatment, based on training, experience, or certification.
- Provides evaluations and assessment for both mental health and substance abuse clients.
- Coordinates with other agency and community providers to meet client needs and insure quality care.
- Case Management Services are provided to clients and their families in conjunction with a client’s treatment plan.
- Care coordination is made available as needed.
- Care Coordination involves deliberately organizing patient care activities and sharing information among all of the participants concerned with a patient's care to achieve safer and more effective care.
- Care coordination seeks primarily to help a patient navigate the system, helping the client to work across care settings with various providers involved in their care.
Case Management
Duties will include but are not limited to the following:
- Monitoring/Follow-up
- Referral
- Advocacy
- Crisis Intervention
- Transportation
- Budgeting
- Life Skills
- Any other service that will assist client in completing average daily living skills.
Other Services
- Participates in agency outreach, education, and consultation activities.
- Maintains positive working relationships with colleagues, other agencies, and community members to insure quality client care, enhance agency functioning, and meet community needs.
- Will provide services on an as needed basis for the Wyoming Boys School. This includes but is not limited to all Clinical duties listed above in an adolescent setting.
Related Duties
- Completes client related records and reports in a timely, accurate, and complete manner.
- Completes accurate time logs on a daily basis.
- Participates in staff and supervisory meetings.
- Attends training as needed to maintain and develop clinical skills.
- Keeps license/certificates current.
- Other duties may be assigned to meet agency needs.
Education and Experience:
- Minimum of 1year of experience required, 2-3 years preferred.
- Bachelor's degree in applicable field
Required Skills and Abilities:
- Strong knowledge of industry terminology.
- Excellent customer service skills.
- Ability to work independently or within a team setting.
- Strong attention to detail.

Pay: $17.31 - $24.04 per hour
Expected hours: 40 per week
