Job Description Summary: Minimum starting salary $25.39 / hour
The Nurse Care Manager (NCM) manages assigned caseloads and champions a holistic, person-centered approach for coordinating the healthcare needs and wellness goals of clients. They provide services to individuals impacted by a variety of behavioral health (addictive/substance use and mental health) issues, recognizing that the individuals we serve often have experienced trauma that affects their development and adjustment. The NCM demonstrates a warm and welcoming empathic, hopeful attitude, conveying a philosophy of dual recovery. They will be trained in trauma-informed care and will work to help provide services in an environment that is sensitive and responsive to client needs that will prevent victimization, abuse, or trauma because of the care received. The NCM reports to the Healthcare Home Director.
Education:
- RN (Registered Nurse) license (may substitute LPN (LICENSED PRACTICAL NURSE) with knowledgeable experience provided)
Licenses and Certifications:
- Must have and maintain a current Missouri license as a Registered Nurse or Missouri Licensed Practical Nurse and maintain current licensure throughout employment.
Responsibilities:
- Effectively coordinate timely care with external healthcare providers, including MO HealthNet contracted pharmacies and providers.
- Proactively identifies and provides follows-up for treatment and medication alerts reported by Health Information Tools (HIT) and communicates algorithm concerns, problems, or suggestions to HCH Director.
- Effectively communicates client health concerns timely with client’s Community Support Specialist (CSS) regarding identified health conditions and provides the CSS with educational training on chronic disease states, health coaching, medications, and healthy living to assist client.
- Completes timely reviews and signs-off on the health screening with the client and identifies measurable health and wellness needs/goals prior to the annual psychosocial assessment and communicates goals to PSA (Psychosocial Assessment) staff or Case manager assigned.
- Completes all required documentation within established timeframes and according to quality standards set by State and Federal regulations, CARF certifications, and program expectations. The NCM will attend and participate in HCH staff meetings, Physician Consultant client meetings, case management team meetings, supervisory meetings with HCH Director, and required meetings and trainings.
- Conducts quality performance activities as established by the program and/or Ozark Center to ensure clinical services are provided with integrity.
- Actively meet identified performance measures as set out by DMH (Department of Mental Health)/ MO Health Net and Ozark Center regarding HCH program and client care list.
- Performs all other duties as assigned
Physical Requirements:
- Typical physical demands include prolonged sitting, frequent standing, bending, stooping, and occasional lifting.
- Requires normal range of hearing and manual dexterity sufficient to operate keyboard, telephone, photocopier, calculator, and other office equipment as needed.
- Requires ability to work under stressful conditions and to work irregular hours and deal with irate individuals.
- Requires use of a computer.
- TB test upon hire.
