We are seeking an experienced MDS Coordinator to join our healthcare team. The ideal candidate will have a strong background in clinical documentation improvement, patient care, and medical management.
*Responsibilities*
- Utilize knowledge of ICD-10 coding and Medicare guidelines to ensure accurate patient assessments
- Coordinate with clinic staff to gather necessary information for MDS completion
- Conduct comprehensive assessments of patients' medical conditions and treatment plans
- Collaborate with healthcare providers to optimize patient care and outcomes
- Maintain detailed and accurate clinical documentation using Epic software
- Implement clinical best practices to enhance the quality of patient care
*Requirements*
- Proficiency in clinical documentation improvement and medical management
- Strong understanding of physiology and acute care processes
- Experience with ICD-10 coding and Medicare regulations
- Ability to work effectively in a fast-paced healthcare environment
- Excellent communication and interpersonal skills
- Experience with Matrix is a plus, but not required
Applicant must be LPN or RN and be able to participate on an on call rotation and assist as charge nurse or direct caregiver as needs arise.
