Reports To: Clinical Manager / Director of Nursing
Job Summary:
The Saturday Home Health Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes, ensuring quality, compassionate care while promoting patient independence. The RN will assess patient conditions, develop and implement care plans, administer medications, and educate patients and caregivers on disease management. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently.
Key Responsibilities:
Patient Care & Assessment:
Conduct comprehensive assessments of patients medical conditions, home environments, and care needs.
Develop, implement, and adjust individualized care plans in coordination with physicians and the home health team.
Monitor and manage symptoms, wound care, IV therapy, and medication administration.
Education & Support:
Educate patients and caregivers on disease management, medication administration, and home care best practices.
Provide emotional support and guidance to patients and families.
Encourage patient independence and self-care where possible.
Coordination & Documentation:
Communicate patient updates and concerns to the interdisciplinary team.
Document all visits, assessments, interventions, and patient responses in the electronic medical record (EMR) per regulatory requirements.
Ensure continuity of care by coordinating with the weekday home health team.
Compliance & Safety:
Follow all agency policies, procedures, and regulatory guidelines (Medicare, state, and federal).
Maintain patient confidentiality and comply with HIPAA regulations.
Adhere to infection control and safety protocols in the home setting.
Qualifications:
Current and unencumbered Licensed Practical Nurse (LPN) license in Missouri.
Minimum 1 year of nursing experience, preferably in home health, hospice, or medical-surgical settings.
Strong clinical assessment and critical thinking skills.
Ability to work independently and manage time effectively.
Excellent communication and interpersonal skills.
Reliable transportation and a valid driver's license.
CPR certification.
Work Schedule & Compensation:
Shift: Saturdays (hours may vary based on patient needs).
Compensation: Pay per visit.
