Since 1983, P.R.I.D.E. has been a world-renowned, research-driven leader in functional restoration, and we are excited to expand! We are currently seeking a Medical Case Manager (RN, Rehabilitation Counselor, or LMSW/LCSW) to join our dedicated team in Ft. Worth.
Position Overview:
The Medical Case Manager will be responsible for guiding patients through goal-oriented decision-making and problem-solving, aimed at improving their overall functioning. This role involves consistent communication with our internal treatment team—comprising physical and occupational therapists, psychologists, counselors, nurses, and physicians—along with external parties such as insurance representatives, rehabilitation nurses, employers, and attorneys, all while focusing on achieving the best outcomes for the patient.
Key responsibilities include:
- Conducting disability and vocational plan assessments
- Assisting with the development of individualized plans
- Ensuring quality assurance tracking throughout the therapy process
- Collaborating closely with the patient to identify and overcome treatment barriers
Key Skills and Qualifications:
We are looking for a Medical Case Manager who excels at identifying barriers to treatment and demonstrates a commitment to teamwork and patient-centered care. The ideal candidate will have intermediate to advanced knowledge of workers’ compensation systems, the medical and psychosocial issues impacting disability, and vocational assessment methods.

Education & Training:
- Bilingual (Spanish) candidates are preferred
- RN or Master’s degree in Rehabilitation Counseling, Social Work, or a related field
- At least two years of experience in disability management, vocational rehabilitation, or medical case management
- Active certifications or licenses such as RN, CRC, CCM, LMSW, LCSW, etc., are highly preferred
